Full Job Description
Over the past forty years, Montefiore’s Division of Substance Abuse has helped thousands of men and women enter recovery and re-join their families and communities. Through recovery, individuals are empowered to help find meaning in their lives and integrate back into their communities. The Outreach Specialist will assist individuals enrolled in treatment at DoSA in engaging with their substance abuse treatment. He or she will aid patients in the program with obtaining abstinence and complying with scheduled appointments, and will help patients engage and comply with their onsite and offsite primary care and psychiatric treatment. The Outreach Specialist will aid in initial patient engagement, outreach efforts and accompanying of patients to appointments as needed.

 

Responsibilities:

Work closely with patients enrolled in DoSA, Next Steps and Project Rising to aid in engagement in treatment. This may include meeting with patients within the program, calling patients by phone, meeting with patients in the community, and accompanying patients to appointments. Outreach patients who have stopped attending treatment at their last know residents
Actively listen to patients enrolled at DoSA, Next Steps and Project Rising and encourage them to communicate any concerns to appropriate staff on their treatment team. Major issues will be escalated to supervisors
Document all patient contacts in the electronic medical record, including phone calls, onsite visits and offsite interactions
Meet with patients in emergency department to assist in engagement and referral to substance abuse treatment at DoSA, Next Steps and Project Rising
Assist individuals enrolled in treatment with referrals to needed services and connection to resources in the community
Describe the nature and purpose of services offered at DoSA, Next Steps and Project Rising to patients interested in or enrolled in the program
Motivate patients enrolled at DoSA, Next Steps and Project Rising to actively participate in their health and recovery
Collaborate with a multidisciplinary team in order to build and maintain positive working relationships with staff at DoSA, Next Steps and Project Rising
Provide emotional support and instrumental assistance to patients enrolled in treatment
Demonstrate ethical behavior and uphold standards
Other related responsibilities as assigned

Qualifications:

High School graduate / GED required, Bachelor’s Degree preferred
Certified Recovery Peer Advocate (CRPA) or CRPA-eligible following completion of internship hours required
Excellent oral, written and interpersonal communication skills.
Ability to provide outreach to patients enrolled in program, find creative solutions to challenges, build motivation in patients using empirically supported strategies including motivational interviewing
Familiarity with Microsoft Word and Excel

The hours are 7:30am -3:30pm and the ideal applicant will need to have the flexibility to work at alternate shifts if needed.
