Job Description

 

Purpose of Position:

Under the supervision of the Social Work Supervisor and in accordance with the Clinic Director, is responsible for the provision of clinical social work services, case management, and psychotherapy to identified patients in the Psychiatry Adult Outpatient Clinic (AOPC).

Duties and Responsibilities:

Conducts or participates in comprehensive intake assessments with clients and significant others in order to obtain adequate case histories; presents intake data to assigned personnel as appropriate and required.

Assesses clients within departmental timelines and in accordance with institutional standards

Administers PHQ9, and CAGE-AID to patients and completes Mini Mental Status Exam

Diagnoses in accordance with accepted diagnostic classification system(s) as appropriate and required

ln collaboration with the patients, medical team, and a consulting psychiatrist; develops and completes individual assessment-based treatment plan(s) using the SMART model and documents on the patient’s medical record in accordance with the departmental policy

Establishes short and long-term goals; identifies plans(s) of activities in conjunction with individual clients and/or area(s) of service as appropriate and required

Provides time-limited psychotherapy (4-6) sessions

Provides weekly group therapy for up to 3 months

Refers patients with serious and persistent mental illness to appropriate clinics for more intensive treatment

Coordinates care for patients requiring services outside of the AOPC

Conducts interviews with patients and/or family member(s) significant other(s) to determine continuing care needs

Compiles social work documentation and its requirements

Provides psychosocial information for patients transferring to other units to ensure continuity of care

Provides supportive counseling, crisis intervention, and grief counseling to patients and their families/significant others as appropriate

Assist patients with adjustment to treatment, including problems of depression, anxiety, denial, and/or anger; noncompliance with the medical regimen.

Provide information, referral, and advocacy to community agencies

Provide task supervision to caseworkers when designated.

Provides direct/task supervision to social work students when designated.

Leads education and support groups for patients and/or families/significant others as appropriate.

Identifies, evaluates, and designs proper safety for victims of crimes/abuse. ie child abuse, elder abuse, domestic violence, rape, and sexual assault.

Maintains and completes statistics of case activities as outlined in the departmental policy.

Monitors his/her performance by reviewing medical records for appropriate documentation.

Performs other duties assigned by supervisor as needed.
Minimum Qualifications

For Assignment to Level II
1. Valid license as an LMSW issued by the NYSED; or
2. Valid license as an LCSW issued by the NYSED.
