NYC Health + Hospitals/Bellevue is America’s oldest public hospital, established in 1736. Affiliated with the NYU School of Medicine, the 844-bed hospital is a major referral center for highly complex cases, with its 6,000 employees including highly skilled, interdisciplinary clinical staff. Bellevue is a Level I Trauma Center delivering around-the-clock care in adult, pediatric, psychiatric and pediatric psychiatric emergencies as well as in the nationally-designated categories of cardiology, neurology, toxicology, and neonatology. In addition to providing comprehensive inpatient and outpatient state-of-the-art care Bellevue is a city-wide medical specialty referral source. Bellevue’s clinical centers of excellence include: Emergency Medicine and Trauma Care; Cardiovascular Services; Designated Regional Perinatal Center and Neonatal Intensive Care Unit (ICU); Comprehensive Children’s Psychiatric Emergency Program; and Cancer Services.
Job Description
Provide biopsychosocial assessment per referral and assessment policy, plan and carry out appropriate interventions
Complete required documentation in timely fashion, according to assigned area requirements and in professional manner(see documentation policy)
Provide ongoing support and psychotherapeutic counseling to patients, families/significant others as well as concrete services at frequency determined by respective assigned area
As indicated, make contact with patient’s families, collaterals, significant others as well as community and medical providers and include/share this information in assessment, documentation and during multidisciplinary meetings.
Collaborate with family, collaterals and providers in terms of treatment, referrals and / or aftercare planning Escorting patients into the community and making home visits as needed
Attend rounds, and meetings with multidisciplinary teams
Advocate and negotiate with related social and community agencies to secure services for patients
Provide caring, nonjudgmental, ethical services to patient
Participate in Performance Improvement Projects
Compliance with all department, corporate and hospital regulations, policies and procedures and conformity to established dress guidelines, time and leave requirements as an essential worker, and need to wear identification badge while on duty
Incorporate universal screening of patients to identify if they are a victim of abuse. Follow hospital policies on interventions regarding child abuse, intimate partner violence, elder abuse, sexual assault and human trafficking, as well as New York State protocols for mandated reporting of suspected child abuse or maltreatment.
Participate in regular supervision and ad-hoc supervision /escalation as necessary to seek guidance from appropriate personal
Perform work related duties with understanding that as a hospital employee, one is categorized as an essential worker, which includes coming to assignment under a variety of circumstances such as inclement weather, pandemic and other extreme circumstances. Other tasks that may be assigned
Monitor and respond to Provider referrals on electronic queue and otherwise
Utilize Allscripts, Careport, Parachute, EPACES, Workers Connect applications for referrals/orders
Complete forms with teams for services
Provide information about community resources for patients with cancer
Participate as part of the multidisciplinary teams in formulating overall treatment and discharge plans
Minimum Qualifications
For Assignment to Level II
1. Valid license as an LMSW issued by the NYSED; or
2. Valid license as an LCSW issued by the NYSED.
