Service Coordinator

Beanstalk Academy is looking to hire Early Intervention Service Coordinators in all 5 boroughs.
The service Coordinator is responsible for comprehensive case monitoring of all care provided by EI professionals including but not limited to physical, occupational and speech therapists as well as special educators, social workers, nutritionists, and psychologists. The SC must also link families to necessary community resources, collaborate with mutual service agencies, and serve as the primary advocate for the family.

Responsibilities:

Provide coordination to an assigned caseload of children receiving skilled care involving Occupational, Physical, or Speech Therapy as well as Special Instruction, Psychological Care, Family Counseling, or Nursing
Service as the Liaison between the skilled care providers, EI Officials & family; arrange effective contact between parents and skilled care providers, when requested and/or when appropriate
Ensure that all assigned children/families are placed with an appropriate professional/agency, that the child/family is served in a timely manner and that services adhere to the child’s plan of care regarding duration, frequency, setting, and type of services rendered
Document accurate records regarding time frame, billable units, and the content of all case management activities.
Maintain current progress notes for all assigned clients including but, not limited to referral information, care placement, the start of care date, ongoing case concerns, lapses in services, changes in the IFSP Worksheet, missed visits, child welfare issues, need for clinical consultation and discharge planning.
Successfully contact, either by on-site case conferences, phone and/or writing, Therapy Staffing Agencies and/or ongoing assigned personnel regarding the ongoing status of each case
Participation in IFSP meetings
The service Coordinator must maintain the required monthly home visits
Must maintain effective and timely communication with EIOD’s
Participate in monthly individual supervision to enhance job performance skills
Work in collaboration with the QA Supervisor and Business Staff to address outstanding issues in regard to provider activity or denials for services rendered but, not paid
Qualifications:

Three (3) or more years of experience working within a pediatric special needs and/or early childhood program providing comprehensive case coordination
or: An Associates degree in a Human Service field and a minimum of two (2) or more years of professional experience providing pediatric case management services and/or health care coordination
or: A Bachelor’s degree in Psychology with a minimum of one (1) year of coordination experience within early childhood, health care, or social service setting.
Must write legibly in at least a 12 point font or larger
Salary: $11 to $12 PER UNIT (A UNIT is 15 minutes)
