Full Job Description
Responsibilities:Understand and explain available NYS Health Home Services to potential members
Understand, differentiate and implement the outreach and enrollment process for bottom up, top down and lead health home referrals which includes home visits to potential members.
Participate in community outreach and engagement events including health fairs
Work collaboratively with The Salvation Army Corps and Social service programs.
Initiates outreach to targeted providers in the community.
Once member is enrolled, must follow up with assigned case manager within 3 days of enrollment to ensure member has been connected to services.
Ensure all top down and lead health home referrals that have not been enrolled are closed in a timely manner.
Ensure all potential members are eligible and appropriate for Health Home services prior to enrollment including medical qualifications, review available data in MAPP and RMA for restrictions and communicating MCO’s and other CMA’s directly to clear the client for enrollment.
Administrative Responsibilities:
All contacts regarding receipt of referral, outreach to the potential member, and intake of enrolled members must be documented in the RMA within 24 hours. Notes must be specific and include comprehensive notes for every contact.
Ensure each enrollment has a signed DOH-5055 (Information Sharing Consent Form)
Attend weekly supervisory meetings facilitated by the Care Manager supervisor
Participate in the agency quality improvement and professional development programs, attending internal and external training courses and committees
Track accurate and up-to-date case lists including closures
Other Responsibilities:
Available for evening and weekend telephone crisis intervention and coverage for other staff as needed
If bilingual, translate for non-English speaking members
Additional duties as assigned
Qualifications:Associates degree in health or human services related fields with 2 years’ experience required; Bachelor’s degree preferred in health or human services related fields..
Bilingual Spanish / English preferred.
Experience or interest working with HIV/AIDS, chronic illnesses, substance abuse, mental illness, and homelessness.
Awareness of and sensitivity to cultural and socioeconomic characteristics of populations served.
Ability to work collaboratively with other professionals.
Ability to set appropriate limits with members and co-workers.
Excellent writing and oral communication skills.
Good management and organizational skills. Basic computer skills required.
Able to work 35 hours per week, Monday through Friday during normal business hours, or as needed to carry out the job responsibilities.
